Public Health Controversies in Switzerland and the United States: Federal State Decision-Making in a Contentious Climate

英文摘要Prevention policies represents a rising public health challenge of the 21th century. Supranational institutions like the World Health Organization or the European Commission have recently acknowledged their crucial importance and are defining corresponding strategies to increase life quality and reduce social and economic costs. Prevention policies mainly address the problem of non-communicable diseases caused by risk factors such as tobacco consumption or obesity, and of HIV contamination and health damages with regard to illegal drug consumption. There is an urgent need for public policies aimed at reducing these diseases at the population level, which calls for more comparative research on how to tackle these issues. Moreover, such policies touches upon the core of critical political choices such as the legitimacy of state interventionism on private behavior and the political definition of socially acceptable behavior. Such debates are under high public scrutiny and elicit vivid partisanship conflicts, which puts the policymaking process under particular pressure. Public health controversies are therefore of high interest for studying the dynamics and the quality of collective decision-making in contemporary democracies. My project addresses this highly topical issue with a qualitative comparative research design aimed at examining the policymaking process regarding prevention policies in Switzerland and the United States. The project focuses on five case studies which are 1) tobacco control, 2) the fight against obesity, 3) the exceptional use of medical cannabis for medical reasons, 4) the legalization of cannabis for recreational use and 5) harm reduction policies regarding illegal drug consuption. Trough comparative socio-historical analysis, the research aims at explaining the final policy outcome depending on the dynamic of the public controversy in both countries. The Swiss-US comparison is highly relevant because of the comparability of their federalist structure on the one side, and their very different policy outcomes on these matters on the other side. The project addresses a set of original research questions aimed at bringing together public health research and political science, which are two disconnected bodies of research that could however strongly benefit from each other. First, my project intends to make a contribution to federalist studies, by examining the multiple venues through which a contentious issue is put to the agenda and treated in multi-level governance systems. Second, the project aims at proposing a theoretical framework that systematically accounts for the role of the executive, legislative and judiciary powers in shaping contemporary policymaking processes. Finally, this highly innovative project proposes a wider theoretical reflection on the changing patterns of contentious decision-making with regard to rising societal challenges.
